Hazel Evelyn Smith, 79, of Big Creek, KY, passed away Friday, March 29th, at the AdventHealth Manchester Hospital. Hazel was born in Beverly, KY, on April 5th, 1944, a daughter of the late Lewis and Myrtle Holland Roark.
Hazel is preceded in death by her husband, Ray Vernon Smith.
She is survived by her daughter, Melissa Smith of Red Bird. As well as the following grandchildren: Blair Wexler, Brian Andrew Jenkins, Alexandra Griffin, Sebastian River Smith, and Griffin Chase Smith.
In addition to her parents and husband, Hazel is preceded in death by her children: Marrin Ray Smith, Joey Allen Smith, and Brian Lewis Smith. As well as several brothers and sisters.
Services for Hazel will be held at 11:00 AM on Friday, April 5th at the Mudlick Church of the Brethren, with Jim Smith officiating. Burial will follow in the Polk Smith Cemetery on Martin’s Creek.
Visitation for Hazel will be held at 6:00 PM on Thursday, April 4th, at the Mudlick Church of the Brethren.
