Shawana Mary Lee Smith age 40 departed this life on Friday December 5, 2014 in Laurel County, Kentucky.
She was born on October 14, 1974 to the union of Robert Eugene Banks and Sharon June Perry Banks.
She was proceeded in death by her mother Sharon June Kilburn, Her grandparents Robert & Magaline Perry Robert & Margaret Banks.
She leaves to mourn her passing her husband Dennis Smith, her father Robert Banks & stepmother Teresa Grubb, her children Dakota Mullins & Dennis Smith, her step son Jody Smith, her grandson Bryson Smith, her brother Bobby Banks, her sister's Latasha Hoskins, Judy Asher & Samantha Napier.
Services for Shawana Smith, APRN, will be on Monday, December 8th, 2014 at the Horse Creek Holiness Church in Manchester, Kentucky in Clay County, located off the Hal Rogers Parkway (Manchester exit 20), eastbound turn right - westbound turn left.
The Horse Creek Holiness Church will be located about 1 mile on the left.
