RICHMOND, Ky. (July 31, 2016) – The Kentucky State Police, Richmond Post is investigating a fatal collision that occurred on KY-52 near Waco, just after 1:00 pm today.
The initial investigation indicates that Douglas R. Tackett Jr., 39 years old of Richmond was driving west on KY-52 in a 2002 Silver Dodge Caravan, when he lost control of his vehicle and struck a guardrail after crossing the oncoming lane of traffic.
Following the collision with the guardrail, the van overturned several times causing the driver to be ejected from the vehicle. Douglas Tackett Jr. was transported to the hospital, where he was pronounced deceased by the Madison County Coroner.
The ongoing investigation is being conducted by KSP Trooper Cameron Allen.
He was assisted at the scene by KSP Personnel, Madison County EMS, Madison County Sheriff’s Office, Waco Fire Department, Madison County Fire and Rescue, and the Madison County Coroner.
